PERMIT CHECKLIST
TEENS
Permit Certificate (DE-964)
VOE (Verification of enrollment) from High School (Valid for 30 days)
Passport / Birth Certificate / Green Card or Valid Visa
SSN card (Social Security)
2 Proof of Address (utility bills)
Take the person whose name is on the bills
If you wear glasses, take that.
If Applicable
